How to register

If you have moved into our practice area (see below) you are very welcome to register with us.Simply bring in your medical card, having completed the relevant sections. We will also ask you to complete a questionnaire (see below).

If you do not have a medical card you can print and complete the two-page registration form below.  We also ask you to print and complete the questionnaire.

We would prefer to have your old medical card because it gives us your NHS number but if you cannot find it please complete this form  (NB 2 pages) Registration form

It is a great help to have answers to all the questions in this questionnaire

 Questionnaire

Once registered your records will automatically come from your previous practice but this may take several weeks. If your previous practice is able to let us have them electronically this process can happen the same day.

Car parking

We do offer some parking at the medical  centre. There is also public car parking (pay & display) adjacent to the medical centre.

 A number of reserved disabled parking spaces  with direct access to the medical centre are available in the public car park behind the medical centre.  Wheelchair access to the centre is not a problem
